I do know what the rules say and they’re fairly ambiguous. The ref can allow play to continue and later issue a yellow card, or he can halt play, issue a card if he wants, and restart with an indirect free kick.

I do, but they're not very helpful. Simulation is considered unsportsmanlike behaviour, which is an offence punishable by a yellow card. When play is stopped to issue a player with a caution, it is restarted with an indirect free kick to the opponent. But what isn't clear is whether play should be stopped to issue the card.
